Patient full name, date of birth, and contact number
Primary presenting complaint and duration
Relevant clinical history and examination findings
Current medications (especially hormonal therapy, anticoagulants)
Previous investigations: urodynamics, pelvic ultrasound, MRI
Previous pelvic or gynaecological surgical history
Medical aid details if applicable
Your contact details for the consultation letter
Level of urgency / specific clinical concerns

Previous imaging and operative notes (particularly for mesh-related presentations) are especially useful and reduce the need for repeat investigations.
