Authorisation:

Patients are responsible for getting their own authorization from their medical aid for their procedure.
Your authorization number AND document confirming all codes that have been approved, must be emailed to the practice no later than 1 week before your procedure date. Should the practice not have the information requested 1 week before your procedure date, the patient will be removed from the theatre list without notice and will have to request a new date for surgery. The practice will under no circumstances contact the medical aid on behalf of the patient. Any information that the medical aid requires (e.g. Script or motivation) will be emailed to the patient directly, who can then follow it up with their medical aid.

Payment of account:

Our practice charges a private consultation rate which is payable on the day of the consultation.

Our practice has a fixed co-payment for all procedures done in the hospital. The co-payment is payable by latest 7 days prior to your procedure date. Patients will be able to claim the co-payment back from their GAP cover after the procedure date.

Appointments not kept:

Appointments not kept will be charged to your medical aid. Should there be no funds available, a statement will be sent to the patient.

Sick certificates:

Please make sure that you receive your sick certificate from the doctor before you leave the office. Sick certificates for in-hospital procedures will not be given before the procedure date. Sick certificates will be emailed to the patient directly the day after your surgery.

Letters / Motivations for 3rd parties:

Should your medical aid require a motivation letter for your booked procedure, our practice will provide the motivation letter as part of the surgery cost. Any additional motivations or forms (for employers, PPS or chronic) that must be completed by the Doctor, will be done at an additional charge which the patient will be held liable for. The turnaround time for such reports is 10 working days.

Prescriptions:

Patients will not be given a script without a consultation. If you were not seen by the Doctor within the last 6 months, you will be required to make a follow up appointment to obtain a script.

Telephone consultations:

Any request to speak to the Doctor, not relating to post-op complications, will be treated as a telephone consult and will be charged at our normal telephone consultation rate.
